1. Letter of application stating the course intended to enroll

2. Official Transcript of Records, Diploma and others

3. Certificate of Good Moral Character (1 from the Principal/Dean or Professor)

4. Affidavit of Support of the person supporting the applicant's study

5. Financial statement from the bank regarding the capabilities of the person funding the study of the applicant

6. Pictures, Passport size (5 copies)

7. Personal History Statement form duly accomplished (5 copies)

8. Photocopy of the Passport
